| Full Forensic Substance Abuse Assessments: $1500 ( 5-10 page document) |
| Assessments: $500-$650 based on state of violation |
| Substance Abuse Education Classes: $75-$$100 hourly Costs vary based on in person or online Zoom classes. |
INSURANCE INFORMATION:
New Life Recovery & Counseling does not bill insurance; however, services may be covered in full or
in part by your health insurance or employee benefit plan. Our counselors will be able to provide
you with a superbill receipt for each session for you to submit to your provider. Payment is due at
the time of service.
Hours:
Monday through Friday 9 AM to 5 PM
Saturday: 9:00 am – 2:00 pm
Evening hours may be available by special appointment
Telephone hours 9 AM to 5:30 PM

Cash, check and all major credit cards accepted for payment.
Cancellation Policy
I require 24 hours advanced notification if you need to cancel a session. For cancellations of less than 24 hours notice, I charge the cost of the session.
